    Multi-specialty outpatient surgery center. 

    Accountabilities / Responsibilities

    The Medical Records Clerk processes and organizes medical records in a timely manner and in compliance with regulatory and accrediting agencies, as well as state and federal laws. The Medical Record Clerk ensures completeness of the record by auditing charts and completing a medical records checklist. Medical Records Clerk handles the storage and retention of charts, responds to requests for medical record, copies and maintains medical record tracking log. The Medical Records Clerk will function within the Center’s policies and procedures, support SCA Values, SCA Vision and SCA Mission.


    Key Responsibilities:


    • Organizes, files, and retrieves patient medical records
    • Files various medical documentation, including operative notes, radiology reports, and lab results
    • Responsible for accessing, maintaining and auditing all medical charts
    • Pulls charts for storage and keeps a log of all charts that have been sent for scanning
    • Generate and prepare/assemble medical records
    • Organize and evaluate medical records for completeness and accuracy
    • Pull and route records to appropriate personnel or department
    • Prepare reports and forms as directed and in accordance with established policies
    • Knowledge of chart control, access and storage in accordance with established policies and regulations
    • Knowledge of medical records regulations and release of health information


    • High School Diploma or equivalent; Associate Degree or equivalent from a two-year college or technical school preferred.
    • Three to five years’ experience in medical office, hospital, outpatient surgery center or related field
    • Working knowledge of medical terminology
    • Computer experience, Excel, Word, Medical Billing Software and Applications
